Farzad Mostashari, M.D.

Dr. Farzad Mostashari is the CEO of Aledade - the largest network of independent primary care practices in the country.

Since founding Aledade in 2014, he has helped the company grow to serve more than 1,500 practices in 45 states, be accountable for the care of more than two million patients, and prevent more than $1 billion in unnecessary health care costs.

He currently serves on the Founder's Council of United States of Care, is a Visiting Fellow at the Duke-Margolis Center for Health Policy at Duke University, serves as co-chair of the Health Evolution Summit and is on the board of directors for Resolve to Save Lives - a global health organization that aims to save millions of lives from cardiovascular disease and infectious disease epidemics.

Mostashari served from 2011-2013 as the National Coordinator for Health Information Technology, where he launched the regional health IT extension center program, the EHR certification program, and the community college workforce program, among others.

Previously, Mostashari served at the New York City Department of Health and Mental Hygiene as Assistant Commissioner for the Primary Care Information Project, which equipped 1,500 physicians in private practice and community health centers in under-resourced communities with electronic health records, and the NYC Center of Excellence in Public Health Informatics. He was a lead investigator in the outbreaks of West Nile Virus and anthrax in New York City, and among the first developers of real-time nationwide electronic disease surveillance systems.
